Attendings: Where should I document Critical Care? … in a Procedure Note!
As AJC says, take credit for the good work that we do – Will file appropriate facility and professional charges (and fulfill attendings Level of Service requirement as max E&M billing)
… Another note for Attendings on Charging/Level of Service: if you are not the discharging attending BUT have opened the Level of Service calculator, the system will pull in your name automatically. What may happen if you sign out to your colleague and they don’t catch it and have continued to provide care/interventions, your name will file with the charge as opposed to theirs. And it will pull in information from a note that’s not yet complete/suggest a lower charge. *Take Home Point* only open LOS if you’re the discharging provider & ensure it’s your name when filing.
*New* ED Behavioral Health Discharge Smartset – thanks to NPD, EPS and the BH team, we have created the ED Behavioral Health Discharge Smartset to automatically incorporate follow-up plans, inclusive of required PCP follow-up within 1 week and Suicide/Crisis/Massachusetts Behavioral Health Helpline information for our patients and families/caregivers. Please utilize for all your BH-related discharges.
Similarly – ED Fracture / Sedation Discharge Smartset to pull in required sedation instructions, Ortho follow-up, etc.
Hyperemesis Panel – medication guidance by EKG / aligned with pathway!
Expected Uploaded images prior to arrival: Can see Rad status from uploaded images from Comm Center communication prior to arrival!
Ordering Home Meds… when was the patient’s last dose?
- In the Home Meds ordering tab under the Orders activity, click Reorder Home Meds
- The first screen will show you the RNs medication history that you can review, but toggle to ‘Reconcile Orders’ or hit the Next button to move to the ordering screen
- Can see the Last Dose by time of day or exact time if compliance documented correctly. Click the + sign to continue standing essential home medications.
